In order to access the DLC, you must defeat the boss Demagrass in the Tomb of Kzar Karos. After that, you’ll be able to unlock the cache hidden inside the Great Skull in the same location. This will grant you an antidote, allowing you to traverse the toxic swamps and reach Mardek’s Ship, where your adventure begins.
